Anthem has released their 2022 Anniversary Month Change form for groups wishing to add Vivity to their offerings. Groups can request to change their anniversary date with the following conditions:
- Group MUST be paid-current to the requested effective date
- Anniversary Change Form should be completed accurately and completely, or it will be returned
- Anniversary Change Form must be submitted no later than the 20th of the preceding month
- Anniversary date changes can only be performed once in a 12-month period
- Once the anniversary date change is made Anthem cannot reverse the change so please ensure the group/case is aware of any rate and benefit changes
The completed form should be sent to Anthem's new anniversary change mailbox: CASmallGroupAnniversaryDateChanges@anthem.com
Please note that the 2022 portfolio is still in review with the regulator, and the form does have Pending Regulatory Approval noted. Any anniversary change outside of January will require approval and justification for the request. See Anthem's Helpful Hints.
