Anthem Blue Cross would like to inform you that the below Prime Healthcare Hospitals have terminated their Commercial Hospital Agreement effective September 16, 2021. These hospitals are no longer part of the Anthem Commercial Network. Going forward, Anthem will generally no longer authorize or approve scheduled or elective admissions to the following hospitals:
- Centinela Hospital Medical Center
- Chino Valley Medical Center
- Encino Hospital Medical Center
- Garden Grove Hospital and Medical Center
- Huntington Beach Hospital
- La Palma Intercommunity Hospital
- Paradise Valley Hospital
- Sherman Oaks Hospital
- West Anaheim Medical Center
Anthem Blue Cross notified physicians who admit to these hospitals of this development so that they can arrange for admission of all members to alternate facilities. For a complete listing of alternate contracted facilities, including general acute care hospitals, ambulatory surgery centers, laboratories and radiology centers, and dialysis centers, to which Anthem Blue Cross members may be directed, please reference the Anthem website www.anthem.com/ca
