Anthem Blue Cross of California Requires ACH Form After Feb. 15, 2021

Groups submitted to Anthem Blue Cross on or after 2/15/21 must use the ACH/EFT form regardless of effective date.  LISI will no longer send a check copy and forward the live check to Anthem.
