Anthem Blue Cross is pleased to announce they have successfully renewed their Commercial and Medicare Agreements with John Muir Health which provides services to members in Contra Costa County and surrounding communities. John Muir Health includes the providers listed below:
- John Muir Medical Center Walnut Creek (050180)
- John Muir Medical Center Concord (050496)
- John Muir Behavioral Health Center (0054O1)
- John Muir Physician Network
The fully executed HMO, PPO, and Medicare agreements have a November 1, 2021 effective date, excluding John Muir Physician Network HMO which has a January 1, 2022 effective date. All contracts run through October 31, 2024.
