- Small Group Employer Application
- Cigna + Oscar bill / statement (80% enrollment required from the C+O bill / statement)
- Employee Enrollment
- 1st months premium
Additional requirements:
Group must be headquartered in CA
Anthem will review groups with high out of state participation on a case-by-case basis.
