Blue Shield of California and Altais have teamed up to offer complex care services for eligible members. These services allow patients who face difficulties traveling to clinics to receive at-home medical attention though in-person house calls or by virtual (online) consultations.
Altais and BT Health will provide in-person care where the member resides, which may include a skilled nursing facility, acute living facility, hospital, or other location. Altais/BT Health also conducts virtual visits with members in any location.
Services include:
- In-person visits in their home or another convenient place.
- Phone or video care visits
- Urgent care during and after hours
- Mental health care
- Coordination with their primary care provider and specialists.
- 24/7 support
Service Area:
This year, members in 10 Bay Area counties with Altais are being offered complex care services. Eligible members in these areas are being notified via mail.

Blue Shield will communicate the upcoming go-live dates for these counties once all information is available.
Offered at no cost:
There is no impact or any change to their health plan or benefit if members participate in the program. Members are free to accept or decline participation. There is no additional cost or co-pay to the member. Note: Covered services provided by non-Altais/BT Health providers through a referral may be subject to co-pays based on the member’s benefits and coverage.
Program Eligibility:
Members are identified solely by Blue Shield at this time. Blue Shield identifies eligible members and Altais contacts the Independent Practice Associations (IPAs) and the primary care physicians (PCPs) of these members. Physicians who received a letter from Altais with the names of members eligible for Altais/BT Health through the Complex Care Program may encourage those members to enroll, following the process outlined in the letter, and to call Altais/BT Health directly.
