Diagnostic Clinic Medical Group (DCMG), a GuideWell Company, provided primary and multi-specialty care for patients in Pinellas and Polk counties. Their patients include 36,957 members that had or have a Florida Blue health plan during the last three years.
All four of DCMG's clinics will be closing. The last day of scheduled patient care will be August 28, 2024, and the clinics will officially close on August 31. Affected patients were sent a letter from DCMG on or about June 18, 2024, informing of the closure and letting them know how to obtain their medical records.
On September 3, 2024, four new Florida Blue | Sanitas Medical Centers will open near or at the current DCMG sites. The new Sanitas centers will provide comprehensive, value-based primary care for Florida Blue commercial members only. They will be located at:
- Countryside: 30535-30728 US Highway North, Palm Harbor, FL 34684 (new location)
- Crosswinds: 6735 Crosswinds Drive North, St. Petersburg, FL 33710
- Largo: 1157 Missouri Avenue North, Largo, FL 33770 (new location)
- Winter Haven: 1020 Fifth Street Southeast, Winter Haven, FL 33880
Members who have a Florida Blue Commercial HMO plan will be automatically assigned to a new primary care doctor at a Florida Blue | Sanitas Medical Center at the site of, or nearby, their current DCMG clinic. Members will be able to make appointments by August 1, but should be aware that the new Florida Blue | Sanitas Medical Centers may not appear in the provider directory until closer to their opening date of September 3.
Members may also find a new provider by visiting FloridaBlue.com or TruliForHealth.com as appropriate; by visiting a local Florida Blue Center; or by calling the number on the back of their ID card. HMO members are reminded that, if they switch their PCP to any provider other than the automatically-reassigned provider, they need to ensure that the new preferred primary care doctor is updated in their Florida Blue member account before they start visiting that doctor. Florida Blue myBlue members are also reminded that, unless it is an emergency, they will pay the full cost of the visit if they don't start with the new provider first, or another provider in the same medical group.
Note: Typically, members have the option of continuing to see an out-of-network doctor for a period of time if they are receiving active care. This is called continuity of care. However, because DCMG is closing, continuity of care will not be available.
