Cano Health, a group of provider practices with locations throughout Florida, will be terminated from the Florida Blue network effective December 31, 2023. Beginning on January 1, 2024, care received from the following providers will be treated as non-network:
- Cano Health, LLC (provider ID #000K1)
- Cano Health PCP, LLC (provider ID #II1F6)
- DGM MSO, LLC (provider ID #S1Z2H; currently has no assigned providers)
This termination, which is a business decision by Florida Blue, will apply to 368 providers in the following counties:
Brevard, Broward, Highlands, Hillsborough, Lake, Lee, Manatee, Marion, Miami-Dade, Orange, Osceola, Palm Beach, Pinellas, Polk and Volusia
A total of 9,619 members will receive notifications in the mail during the first week of October, 2023. Employer groups throughout the impacted counties will also be notified. Note: groups on the Sales "Do Not Mail" list will be excluded from the notification mailings.
PCP Reassignments: no PCP reassignments will be done initially. During December, Florida Blue will determine if any members remain with Cano Health PCPs and will reassign at that time.
Member Outreach: Florida Blue service teams will begin reaching out to impacted members and setting appointments during open enrollment. Members can also find other in-network providers by visiting FloridaBlue.com or calling the customer service number on their member ID card.
Continuity of Care (COC): Florida Blue will analyze medical claims from the past 90 days, to identify which members may be in active treatment for a serious or complex medical condition (or pregnancy) that qualifies for continuity of care. Florida Blue will work with Cano Health to minimize potential disruptions for these members, by implementing continuity-of-care processes that conform to state and federal guidelines.
