Florida Blue is simplifying Agent of Record (AOR) change requests effective immediately.
Florida Blue previously required the group’s written request to be submitted on their company letterhead with the authorized Group representative’s signature. However, requests no longer need to be submitted on group letterhead!
With the new process, a group's authorized representative may submit the request. Authorized representatives include:
- Business decision maker (as listed within Florida Blue)
- Benefit Administrator or additional group contact (as listed within Florida Blue)
- Company officer (as listed within the State of Florida’s Division of Corporations website)
Request requirements:
- Group Information: group name and number
- Receiving Agency Information: agency name and AOR number
- Receiving Agent Information: agent name and AOR number
The Group AOR Change Form may be used to capture this information. The information can also be included in the body of the email.
Send the requested information and the original email request from the Group’s authorized representative to ChannelPartnerDataManager@bcbsfl.com for Small Group.
