- For groups with 5+ enrolling. 25% participation. Waivers required. No DE9C, payroll, ownership documents or prior carrier bill required.
- Sell HMO with 6 Promo through June 2024
- Mix and match any HMO plans. For groups with 6+ enrolling employees. No minimum participation requirement. No DE-9C, payroll, ownership documents, prior carrier bill, or participation attestation required. No waivers required when not paired with life or employer paid dental/vision.
