Health Net's simplified underwriting has been extended through Q3
- Enhanced Choice| 5 enrolled. DE-9C, payroll, prior bill and ownership documents not required. Only need enough waivers listed on census to meet participation.
- HMO with 6| Mix and match HMO plans with 6 enrolled. Waivers*, DE-9C, payroll, prior bill, participation agreement and ownership documents not required.
- Start-up Groups| Down to 1 enrolled with 2 weeks payroll (either 2 weeks before effective date or after effective date.
- PEO sub-groups| Eligible with DE9C or 2 weeks of payroll from the PEO.
- Slice groups| Write Health Net alongside any carrier, no limit to the number of carriers when participation is met.
- Easy participation| 25% for 5-100 enrolling. 70% for 1-4 enrolling.
