Kaiser Permanente® just announced it is partnering with the Mayo Clinic to invest in Medically Home, an innovative company with a one-of-a-kind technology and service platform that enables caring for patients at the higher end of the clinical acuity spectrum safely in their home.
Medically Home is at the leading edge of the transformation of the delivery of care for patients with serious and complex care needs. As many as 30% of patients treated in hospitals can safely receive care in their homes, potentially impacting millions of patients. Medically Home aligns with Kaiser's values of putting patients first.
The pilot program launched in Northern California, with plans to expand soon.
