You and your clients have enough to think about. Kaiser® keeps tax filing simple by providing easy access to 1095-B forms and information on what to expect next.
Subscribers have the ability to access their 1095-B from Kaiser Permanente through kp.org. You must be a registered member to access this secured feature.
Members who will receive Form 1095-B from Kaiser Permanente include:
- Small group
- SHOP
- Large group (fully insured)
- Off-Marketplace individual and family plan members
For subscribers receiving a 1095-B from Kaiser Permanente:
- Form 1095-B will be mailed to subscribers from Kaiser Permanente by early February.
- Form 1095-B may be used to support proof of coverage on your tax filing. You may need your form to show proof of medical health plan coverage when filing your tax returns with a tax preparer. Please visit the IRS page on health care information forms for more information.
- If you believe the information on your Form 1095-B is incorrect, please contact Kaiser Permanente at 844-477-0450. The hours of operation are from 8 a.m. to 6 p.m., Monday through Friday, and 7 a.m. to 3 p.m. Saturday and Sunday (Pacific Time).
- If you lost or need another copy of your Form 1095-B, the form is available through your personal documents page.
