As of January 1, 2019, the health insurance coverage for some Sharp Health Plan members will end. Sharp Health Plan will no longer offer health coverage to members who live in the following ZIP codes: 91934, 92004, 92036, 92066. Members will be able to remain enrolled in Sharp Health Plan if their employer is located in other ZIP codes within Sharp Health Plan’s service area.
For your reference, Sharp Health Plan has included the Broker-Sample-Enrollee-Notice that they will send to affected Off-Exchange members.
