Simple Enrollments with Delta Dental

In an effort to make the enrollment process as simple as possible, Delta Dental has agreed to accept the Delta Full Census to satisfy the DE9C and employee enrollment forms and waivers requirements.

This means that groups applying for Delta Dental PPO or DeltaCare HMO can submit just the two documents plus payment to CoPower:

1. the Employer Application
2. the Delta Full Census
3. the Electronic Debit Payment Form (or company check)
