Health benefit taxes and reporting requirements can be confusing to small businesses
To simplify IRS reporting requirements for small businesses, a 6055 Membership Report will be available for groups to complete their 6055 form filings. Prepopulated Forms 1095-B for all covered members will be available for eligible small group*. Because this service is optional, groups can access their documents by logging into their plan sponsor accounts.
If an Applicable Large Plan Sponsor is subject to additional tax responsibility, the 6055 Membership Report can be used when completing form filing requirements. Determination of an Applicable Large Plan Sponsor is the responsibility of the plan sponsor.
Group contacts will receive email notification shortly with instruction on how to obtain their documents.
Note: Plan participants that reside in New Jersey, California, Vermont, Rhode Island, and Washington D.C. at any point during 2022 may be subject to penalties if form filings are not completed accurately and timely.
All Savers
A copy of the 6055 Membership Report will be available for your All Savers Alternate Funding groups on the myallsavers.com broker website in the groups Company Master Record. To access a copy of the report:
- Log into your broker account on myallsavers.com
- Select the Search tab along the left of the screen and search for the group
- Select the group entry with a case type of “Company Master Record”
- Select the Documents tab along the top of the screen
- Select IRS REPORT to expand the section
- Select the most recent document available to open a copy of the group's 6055 Membership Report
Level Funded
A copy of the 6055 Membership Report and prepopulated Forms 1095-B will be available for your UnitedHealthcare Level Funded or Oxford Level Funded groups on the uhceservices.com broker website in the Reports tab. To access a copy of the report:
- Select the Reports tab at the top of the screen
- Select Level Funded
- Search for your group using either the Group ID or Group Name
- Select the most recent IRS Documents 6055 or IRS Documents 1095B for the individual prepopulated Forms 1095-B
* Eligible small groups include those reported to have less than 50 medical lives currently enrolled.
To prepare these forms, we rely on the information provided by the plan sponsor, broker, and members. Completeness and accuracy of the Forms 1095-B will depend on whether the information provided is complete and accurate.
The preparation of the Forms 1095-B is a service provided to small groups, the form and the contents within remain the sole responsibility of the plan sponsor. In using the prepopulated forms, the plan sponsor acknowledges and agrees that any IRS risk or issue will be the sole responsibility of the plan sponsor.
