Medical Loss Ratio must be calculated and reported for all fully insured individual and group business, with customers aggregated by group size, legal entity and state. UnitedHealthcare has systems and processes in place to gather the data necessary to calculate medical loss ratio and determine the eligibility of policyholders for rebates.
