UnitedHealthcare Level Funded
Finding an affordable health care solution for small groups may be challenging. UnitedHealthcare Level Funded is designed to help meet the challenges of rising health care costs while providing freedom and stability with a Level Funded health plan. UnitedHealthcare Level Funded can give your clients more options, including access to the large proprietary UnitedHealthcare network and the Optum Rx network of pharmacies.
What are the requirements for participation?
- 2 eligible – 2 must enroll
- 3 eligible – 3 must enroll
- 4 eligible – 4 must enroll
- 5 to 8 eligible – Minimum of 4 must enroll
- 9+ eligible – 50% participation required
Groups with only 2 enrolled employees must include at least 1 common law employee who is not the owner’s spouse.
What are the details of the program?
- Standard enrollment documents required.
- Late applicants not eligible; member enrollment after effective date is subject to a qualifying event.
- Reporting is not available for this segment size.
United Healthcare Rewards
This is a wellness program built to inspire healthier habits. With Rewards, healthy choices may result in healthy savings.
Participants track daily activities designed to help them move more and take healthy actions, with the potential of getting rewarded up to $1,000 — depending on their plan.

- Offering a registration incentive for completing onboarding questions and pairing a device.

Care Cash
Studies show that 72% of consumers are concerned about health care and out-of-pocket expenses. To help employees pay for certain health care costs and encourage them to use quality care, we offer Care Cash®. This is a preloaded debit card that helps guide you to eligible network* care (and helps pay for it). Care Cash® comes loaded with $200 for individual or $500 for family plans.
Plus, the card is reloadable, with any remaining balance rolling over each year.
What can I use Care Cash for?
- UnitedHealth Premium® Care Physicians - Doctors who meet benchmarks — based on national standards — for quality and cost efficiency (from specialists to family clinicians). When searching for a Premium Care Primary or Specialist provider on myuhc.com® or the UnitedHealthcare® app, look for 2 blue hearts near the doctor’s name. These providers include Primary care* (routine care from physical to behavioral health) and Specialty care (for specific concerns from cardiology to orthopedics).
- Primary care - A primary care provider (PCP) is the doctor who may know you best — they’re the first person you call with medical questions and issues. PCPs can help guide you to the care you need — and may help you avoid surprise costs, save time, and make the best use of your health care.
- Urgent care - From care at convenience clinics (for flu shots and minor injuries) to urgent care (for broken bones and infections). Urgent care clinics offer treatment for non-life-threatening injuries or illnesses, like sprains and minor burns. These clinics are staffed by physicians and care teams.
- 24/7 Virtual Visits - Virtual urgent care for common concerns (from the flu to allergies). When you need care — 24/7/365 — Virtual Visits may be a convenient option. You can connect with a doctor by phone or video through your mobile device, tablet or computer and get treatment for allergies, headaches, rashes and more — from wherever you are.
How does Care Cash work?
- Employees receive information about Care Cash and can request the card on myuhc.com®.
- Once received and activated, the Care Cash card can be used for certain eligible health care expenses.
- Employees have access to viewing other programs and benefits through myuhc.com® or can call a toll-free number for guidance.
