UnitedHealthcare Updates Scheduled Direct Debit Forms
UnitedHealthcare has issued new Scheduled Direct Debit (SDD) forms. These new forms can be used immediately; old versions will be rejected as of April 1, 2021.
Verbiage has been added to the Statement of Understanding on page 2, which lets employers know that by submitting the form, they are electing to receive all invoices online only.
There continues to be 3 versions of the form depending on the elected coverages:
