It's that time again! In preparation for annual tax reporting, your employees will likely ask for their Form 1095-B. While members may not need to submit this form with their tax returns, they may need a copy to show proof of health coverage when meeting with a tax preparer.
In compliance with Federal law, Western Health Advantage provides enrollment information to the IRS automatically. Members can access their copies of the forms by doing the following:
- Form 1095-A: Applies to Covered California members only. Please contact Covered California at (800) 300-1506.
- Form 1095-B: Members can log in to their account at MyWHA.org and find a link to the form under MyDOCUMENTS. They can also call Member Services at (888) 563-2250 to request a form be sent.
- Form 1095-C: This form is employer-provided. Members should contact their employer.
