Note: If the Q1-Q3 2022 totals are not met, they can be made up in Q4 '22 & Q1 '23. Production from Q1-Q3 '22 cannot be used to hit the Q4 '22 & Q1 '23 - $105k, i.e., any Q1-Q3 ’22 surplus above the target doesn’t carry over to meet the overall total.

For Connecticut, New York, Maryland, DC, Virginia & Arizona

Other Qualifications

• 3 medical cases
• Book transfers based on creditable medical business, and Amwins gets paid by the carrier
• All production must be creditable, i.e., a sold ASO case does not qualify
