If the client receives a survey from their carrier, it must be completed and returned to the carrier by the documented deadline. If the completed survey is not returned to the carrier, the client will be responsible for providing the required information to CMS. The survey will be asking for plan sponsor demographic information and other data the carrier may not have in their system.
