Updated Information on the Federal CHIP Model Notice
The Federal Children’s Health Insurance Program (CHIP) Model Notice has been updated as of March 17, 2025.
Children’s Health Insurance Program Authorization Act requires employers to provide an annual notice to their employees if the employer and/or employees resides in any of the states listed in the notice.
The notice provides information for states that provide premium assistance subsidies under a CHIP or Medicaid plan if the individual resides in a state listed in the model notice. The programs are based on where the plan’s eligible individuals reside, not the employer’s physical location.
38 states offer the CHIP program or a Medicaid program to assist individuals.
